Utah Cosmetology License Requirements

The Utah Cosmetologist/Barber license is the state's comprehensive full-service cosmetology credential, authorizing hair, skin, nail, and lash and brow services. It requires 1,600 hours of approved training and passage of NIC Theory and Practical Examinations. The following steps reflect the process under Utah Code Title 58, Chapter 11a and Administrative Rule R156-11a.

Steps to Get Your Utah Cosmetologist/Barber License

1. Enroll in a DOPL-licensed cosmetology/barber school. Complete your training at a Utah-licensed cosmetology/barber school or a nationally accredited school whose credits Utah must accept under R156-11a-302c. Utah law does not specify a minimum age for cosmetology licensure. Alternatively, register as an Apprentice ($20.00 fee) and complete 2,500 hours of on-the-job training under a licensed supervisor at a licensed school. Note: apprenticeship hours and school hours cannot be combined or interchanged.
2. Complete 1,600 hours of curriculum training. The Utah cosmetology/barber school curriculum is defined in Administrative Rule R156-11a-705. It covers hair services, barbering, basic esthetics, nail technology, lash and brow work, chemistry, anatomy, sanitation, and infection control. Your school tracks and certifies completed hours. Schools must accept transfer credit hours from other approved programs up to the maximum allowed under R156-11a-302c.
3. Submit your license application to DOPL. Apply online at utahdoc.mylicenseone.com (available as of January 31, 2026) or submit the manual Cosmetology Professions License application form by mail. The application fee is $60.00. Include verification of graduation from your licensed school. All examinations must be passed within one year of this application date.
4. Register for the NIC examinations through PROV. Once your school certifies graduation, register with PROV at provexam.com for the NIC Cosmetology/Barber Theory and Practical Examinations. PROV administers all NIC examinations on behalf of DOPL.
5. Pass the NIC Theory and Practical Examinations. Both the theory (written) and practical examinations must be passed with a minimum score of 70% or higher. Scores are submitted directly to DOPL by PROV. You have one year from the date of your application submission to pass both examinations.
6. Receive your Utah Cosmetologist/Barber license. Once DOPL confirms passing scores and all application requirements are met, your license is issued. Your license will expire on September 30 of the next odd-numbered year (e.g., September 30, 2027). Renew through utahdoc.mylicenseone.com before that date.

Applying by Endorsement (Out-of-State Licensees)

If you hold an equivalent cosmetologist/barber license in another U.S. state or territory, you may qualify for licensure by endorsement. Submit the endorsement application at utahdoc.mylicenseone.com with official license verification from the other jurisdiction. You must still pass the NIC Theory and Practical Examinations unless a waiver applies. Military Applicants

Utah offers expedited licensing review for active-duty military members, veterans, and military spouses. Review the Military Resources page at commerce.utah.gov/military-resources/ before submitting your application.

Internationally-Trained Applicants

DOPL evaluates international training on a case-by-case basis. A Limited Supervised Training Permit ($50.00 application fee) may be available for internationally-trained applicants who need additional supervised training hours before qualifying for full licensure. Utah training hour requirements are set by Administrative Rule R156-11a, which also defines the required curriculum content for each school type. Training must be completed at a DOPL-licensed school or through an approved apprenticeship program. Apprenticeship hours are higher than school hours because apprentices receive training on actual clients rather than in a structured curriculum environment.

Training Hours by License Type

Curriculum Content

Each school's curriculum must cover the subjects defined in the corresponding administrative rule section. The cosmetology/barber curriculum (R156-11a-705) includes hair services, barbering, basic esthetics, nail technology, eyelash and eyebrow technology, chemistry, anatomy, sanitation and infection control, and Utah state laws and rules.

Transfer Credit

Under R156-11a-302c, a licensed Utah school must accept transfer credit hours from other approved programs up to the total hours required for graduation. The school evaluates the student's level of training before assigning credit. Hours earned in an apprenticeship program cannot be used to satisfy school instruction hour requirements, and vice versa.

Credit Hour Conversion

For programs measured in credit hours rather than clock hours, Utah's rule at R156-11a-102(10) defines the conversion: theory and practice credits equal 30 clock hours per credit hour in standard programs and 20 clock hours per credit hour in certain accelerated formats. Clinical experience credits equal 45 or 30 clock hours per credit hour, depending on program structure.

Utah requires passage of National-Interstate Council of State Boards of Cosmetology and Barbering (NIC) Theory and Practical Examinations for all cosmetology-related licenses. Examinations are administered by PROV (provexam.com) under contract with DOPL. All examinations must be passed within one year of the date the license application is submitted to DOPL.

Examination Modules by License Type

Passing Score

A minimum score of 70% or higher is required to pass both the theory and practical components for all Utah cosmetology-related examinations (R156-11a and DOPL FAQ). Examination results are submitted directly to DOPL by PROV after each testing session.

ICA Permit Exam (Specialty Permits)

Applicants for the Hair Safety Permit and other specialty permits complete an Integrated Competency Assessment (ICA) permit examination, also administered through PROV. This examination is specific to the permit's authorized scope of practice. No DOPL fee is charged for the Hair Safety Permit; the testing fee is paid directly to PROV.

Utah cosmetology, barbering, and esthetics licenses expire on September 30 of odd-numbered years. All licenses in the same renewal cycle expire on the same date, simplifying tracking for practitioners who hold multiple Utah licenses.

Renewal Steps

1. Watch for your renewal notice. DOPL mails a renewal notice to the licensee's address of record at least 60 days before the September 30 expiration date. Keep your mailing address current in the DOPL system. Update your address at commerce.utah.gov/dopl/update-your-license-information/.
2. Log in to the online portal. Renew at utahdoc.mylicenseone.com. This portal replaced the previous DOPL online system as of January 31, 2026. Manual renewal forms are also available at commerce.utah.gov/dopl/cosmetology/renew-a-license/ for mail-in or in-person renewal at the Heber Wells Building.
3. Pay the renewal fee. Renewal fees are $52.00 for most practitioner licenses, $68.00 for Master Estheticians, $32.00 for Electrologists, $20.00 for Apprentices, and $110.00 for Schools. There is no continuing education requirement to complete before renewing.
4. Receive your renewed license. DOPL processes renewals and issues confirmation through the portal. Save your renewal confirmation as proof of active licensure.

No Continuing Education Required

Utah does not require any continuing education hours for cosmetology, barbering, esthetics, nail technology, or electrology license renewal. Administrative Rule R156-11a does not include a CE requirement for practicing licensees.

Late Renewal and Reinstatement

A $20.00 late fee applies to renewals submitted after the September 30 expiration date. Licenses that have lapsed beyond the grace period require a $50.00 reinstatement fee. To check the status of a lapsed license, use the license verification tool at secure.utah.gov/llv/search/index.html.

Instructor License

Instructor licenses automatically renew with the renewal of the licensee's underlying practice license (e.g., cosmetologist/barber, esthetician, or nail technician). No separate renewal application or additional fee is required for instructor licenses.

Utah issues more cosmetology-related license types than most states, allowing practitioners to choose a credential that matches their specific scope of practice. Training hour requirements are from Administrative Rule R156-11a; fees are from the DOPL Fee Schedule (FY 2025–2026).

Full-Scope Practitioner Licenses

Apprentice Registration

Instead of attending a licensed school, applicants may register as an Apprentice ($20.00 registration fee) and train on the job at a licensed school under a licensed instructor. Apprenticeship hour requirements are higher than school hours (e.g., 2,500 hours for cosmetologist/barber vs. 1,600 school hours). Apprenticeship hours and school hours cannot be combined or interchanged.

Specialty Permits

Utah also issues restricted-scope specialty permits for practitioners who want to offer a limited set of services. Each permit requires its own NIC or ICA examination through PROV. All specialty permits have a $60.00 application fee and $52.00 renewal fee, except the Hair Safety Permit (no DOPL fee — tested through PROV).
